Suspicious moles

Over the past decade with mounting evidence of skin cancer almost epidemic, the medical community and the public have responded by removing untold suspicious moles in an attempt to arrest the development of malignant melanoma. However, the increasing incidence and death from the disease continues, with more and more moles recommended for removal.

A few published facts in the peer-reviewed medical literature include the average dermatologist evaluation in successfully identifying moles that were converting to malignant melanoma has been 1 out of 18 cases. With dermoscopy (a tool to clinically improve detection) the successful identification of such moles was 1 out of 4.

The statistics become far worse for non-dermatologists. In other words, dermatologists are batting 0.050 in correctly identifying these moles clinically. Whole body examinations, by identifying mole patterns, reveal between 72-94% of moles depending on age to be consistent in a given individual and not indicative of cancer. Patients under 50 who’ve developed moles were diagnosed with less than 1% of their lesions as malignant melanoma where as patients older than 50 years of age had a 30% incidence of melanoma in new lesions.

This makes it difficult for the patient to decide what to do. Faced with a changing mole or lesion, first seek a dermatologist for evaluation since their understanding far exceeds other specialists. Find a dermatologist, with known expertise in melanoma. Full body exams are also necessary to evaluate changing. Try to obtain a second opinion before letting someone take a scalpel to your body because a “lesion” is suspicious.

It’s also worth remembering most “dysplastic nevi” will never become malignant. The art of evaluating the degree of dysplasia requires intelligence and experience.

A new study approved by the FDA is currently underway in the USA attempting to investigate the feasibility for the use of ReCell in the treatment of hypertrophic dyspigmented scars. Twenty patients with scars will be assessed for healing and pain on a weekly basis during a four-week period, followed by 12 and 24 weeks. The study is designed to assess the use of ReCell for the treatment of existing scars in a single treatment.

Dr Dutta of the Aesthetic Beauty Centre said, “Although ReCell has been available in the North East, at the Aesthetic Beauty Centre for quite some time, many people are still unaware how ReCell can treat conditions such as Vitiligo, skin affected by sun damage, or Hypopigmentation, scars and grade III or IV acne scarring. ReCell allows us to collect skin cells from the patient’s own skin, which can then be sprayed onto the area needing treatment. The cells multiply, spreading throughout the skin resulting in faster healing and improving skin texture and colour pigmentation.”

UK obesity report

A recent report reveled one-third of adults in England are projected to be obese by 2012, according to a report published in the Journal of Epidemiology and Community Health.
In men, the prevalence of obesity between 1993 and 2004 nearly doubled (from 13.6 percent to 24 percent), while prevalence in women increased from 16.9 percent to 24.4 percent during the same period. Dr Dutta of the Aesthetic Beauty Centre commented, ‘Even though a number of weight loss procedures are available at the Aesthetic Beauty Centre including: BIB, SmrtLipo, Vaser Liposelction, BodyTite, Laser Lipoysis and Liposculpture, with the prevalence of such obesity expected to increase in the near future, it’s vital we promote the management and prevention of obesity, and also try to remove the social class inequalities in health’.

Botox in Newcastle upon Tyne and Sunderland

Recent research now suggests patients receiving Botox treatments can reduce wrinkles with half as many sessions after 20 months.

The research, sought to determine whether less frequent Botox treatments could provide longer-lasting reduction of glabellar rhytids. The study included 50 women ages 30 to 50 who received regular Botox injections for two years.

The frequency of treatments can be changed for patients who receive Botox Cosmetic injections every four months for two years, to six months with comparable wrinkle-reducing results and high patient satisfaction.

While a treatment schedule of every three months is likely to achieve the best results in the least amount of time, the study concluded that other schedules offer similar results at less cost.

Bruxism is a habit that effects around 8-10% of the population. It is broadly characterised by grinding of the teeth and clenching of the jaw that causes tooth wear and breakage, disorders of the jaw (pain and limited movement) and headaches. Bruxism occurs in both children and adults but is most common in 25-44 year olds. However, most people grind and/or clench their teeth occasionally to a certain degree.

Bruxism is classified into awake bruxism and sleep bruxism. Awake bruxism is characterised by involuntary clenching of the teeth and jaw bracing in reaction to certain stimuli. There is generally no tooth grinding with awake bruxism. Sleep bruxism is characterised by automatic teeth grinding with rhythmic and sustained jaw muscle contractions.

Many studies have found that there are other characteristics associated with sleep bruxism because it rarely occurs alone. Sleep bruxism occurs as a response to arousals during sleep (periods of awakening), indicating that it may be a sign of a sleep disorder. Nearly 80% of bruxism episodes occur in clusters during sleep and are associated with these arousals. The strongest association has been found between sleep bruxism and Obstructive Sleep Apnoea (periods of stopping breathing during sleep), which is a condition that is often accompanied by daytime sleepiness and non-restorative sleep. Individuals with OSA have many arousals during the night due to their breathing difficulties. The termination of the apnoea event is often accompanied by a variety of other events such as snoring, gasping, mumbling and teeth grinding. OSA has been found to have the highest risk factor for tooth grinding during sleep than any other sleep disorder.

Significant associations with sleep bruxism have also been found with other sleep conditions such as sleep talking, hypnagogic (state of consciousness between sleep and wakefulness) hallucinations, violent or injurious during sleep and REM sleep disorders. Psychological disorders such as stress and anxiety are also known to exacerbate teeth grinding during sleep. In one study, around 70% of sleep bruxists related their noctumal teeth grinding to stress and anxiety. Bruxism is also more prevalent in individuals who regularly use alcohol, tobacco and caffeine (6 cups or more per day).

There is no specific cure for bruxism and it is important to manage the consequences of the disorder. Various preventative measures including mandibular advancement devices, drugs, stress management and occlusal splints have been used. However all but occlusal splints have demonstrated adverse effects which reduce their appropriateness. Recently, Botox has been successfully used to control some of the symptoms of Bruxism. There is no specific cure for bruxism and it is important to manage the consequences of the disorder. Various preventative measures including mandibular advancement devices, drugs, stress management and occlusal splints have been used. However all but occlusal splints have demonstrated adverse effects which reduce their appropriateness. Recently, Botox has been successfully used to control some of the symptoms of Bruxism.

FaceTite™ Treatment

The FaceTite hand piece is designed to create non-excisional, minimally invasive skin tightening of the brow, face, neck, and body skin. FaceTite matches the specific characteristics of the face and body skin envelope, having a thin and superficial fat layer, as well as sensitive structures such as the facial nerve. The hand piece has two RF electrodes – the external electrode is 1.3cm diameter and the internal electrode is 1.3mm diameter (1.6mm with the Teflon) and 10cm long. The internal electrode is a solid rod, thus the hand piece is not designed for aspiration.

Common treated areas include: Lower eyelid, cheeks, nasolabial folds, upper lip, jaw line, and forehead. Occasionally, a neck with little fat may be treated by FaceTite. Any other small body area that needs mainly tightening may be also treated, such as the supra-umbilical abdomen, the inner thigh, or arm skin and cellulite in loose skin.
The whole procedure is performed under local anaesthetic. After 20 min to allow for full anaesthesia, the internal electrode is inserted through the syringe hole or through a tiny scalpel incision. The incision ports may be commissural for upper lip or perioral zones, periorbital for lower eyelid or nasolabial folds, sublobular for jaw line, and in the hairline for forehead.
No compression is applied to the face post treatment and the edema may last for a few days, up to 2 weeks. We recommend a neck compression garment for 5 days when the procedure is performed on the neck and jowels.
Combination treatment of FaceTite with other surgical or non-surgical modalities, with short or no time interval is possible.

Resurgence of Recell skin procedure as treatment for scars

Recell is an innovative procedure which helps to heal damaged skin while improving skin texture and colour using the patient’s own cells.
There has been a recent shift in the main use of this procedure to treat scars resulting from burns, acne, skin cancer and surgery.
The person’s own skin cells are harvested and then processed before being sprayed at the site to regrow and help the skin to heal. In burns cases the procedure is increasingly being used to work in harmony with skin grafts.
The procedure can have profound results for patients both physically and psychologically.
How it works
Dr Ash Dutta of Aesthetic Beauty Centre collects a skin sample from the patient and then processes it. The cells are made into an emulsion and then applied to another area of skin to help improve healing, skin texture and skin colour.
The procedure uses a 'spray solution' which contains all of the skin cells necessary to promote healthy skin growth. This includes keratinocyte cells that form the basis of healthy skin and melanocyte cells that produce skin colour (melanin).
The doctor sprays the harvested cells on to the recipient site. Once applied to the wound, these cells multiply and spread throughout the affected area, resulting in quicker healing.
Recell is used to treat:
• Scars resulting from trauma eg burns, acne, skin cancer or surgery
• Vitiligo, a chronic disorder that causes depigmentation of patches of skin.
• Sun damaged skin
• Hypopigmentation, a condition which causes loss or reduction of skin colour

During the Procedure
A small sample of skin (biopsy) is collected from an area close to the skin you want to treat. This ensures the treated area has the same texture and colour as the surrounding skin. The biopsy is usually around 1-2cm square and is also treated with the spray to help it heal properly.
While the doctor is preparing the area to be treated, the recell procedure enables the doctor to separate the cells allowing them to be easily collected and then sprayed onto the skin.
The treated area and biopsy site will be covered with a dressing, which will then be removed 5 days after the procedure.
The results
The results following surgery with Recell are highly dependant on the reason for undergoing the treatment, the severity of the condition and the ability of the patient’s body to recuperate after an operation. It is normal for the pigmentation to take some time to reach its optimum state but the results can have amazing physical and psychological effects.

How it helps burns scars?
The grade II or III scars do better with Recell. The earlier in the scarring process that the treatment can be instituted, the better the outcome. Healing takes about 7-10 days and the best results are visible in 2-3 months for scars and 8-12 months for pigmentary disorders.

Hundreds dawned their posh frocks and bow ties to make a real night of it. The event is one of the North East’s main corporate showpieces helping to raise thousands of pounds for charity. Housing a huge marquee within the stunning grounds of the County Durham hotel, delights of the sea included, lobster, crab, salmon and Dublin Bay Prawns, followed by a bubbly champagne reception… not to mention giant strawberry pavlovas!

Doctor Ash Dutta and his team at the Aesthetic Beauty Centre, have recently ran a series of advanced training days.

Teosyal, which is a unique anti-ageing treatment used to help hydrate the skin and boost skin tone, was just one of the many dates on the summer/autumn calendar. Nurses and dentists from across the country attended the workshop, to learn how Teosyal could help their patient’s correct lips, cheeks and chins.
